Local Contract 2016
Open this document and complete the highlighted fields. Use the Tab key to move from one required field to the next. Print one copy. After printing, there are several places both you and your parent/guardian must sign and date, which are pages 11, 13, and 16. Both you and your parent/guardian must initial and date the bottom of every page, 1 through 17. In addition, the document must be notarized on pages 11 and 13. Submit the signed and notarized copy to the Local Executive Director prior to the pageant.
