IMPORTANT: After you have qualified (made the $100 donation for one pageant or a maximum of $400 for all pageants entered) you must copy off the receipt and submit it to the local director.

After printing, there are numerous places both you and your parent/guardian must sign or initial and date, which are pages 15 through 21. In addition, the document must be notarized on page 15 and possibly 16. Submit to the local director prior to the pageant.

If you are under 18, your parent must also sign and date. Give to your local director prior to the pageant. In addition, there may be pageant specific forms that a local pageant requires for entry to their specific pageant.
